Our Post-16 Transition programme helps young people take their next step with confidence, whether that’s into college, sixth form, or an apprenticeship. We provide tailored one to one support to help them with the next steps of their journey in life.

What Our Young People and Carers Are Saying
Kim* is being given support in becoming more confident with their abilities and with communication skills when out in public. They are building positive relationships with staff which motivates them to take part in learning and physical activities. Kim* is more motivated to do well academically, in order to reach their goals. They are more confident within themselves and their abilities and doesn’t struggle as much with self regulating their emotions.

“Alex* has gained in self confidence, resilience and personal relationships, along with his academics. A previous bad experience has been nullified (almost) with the exceptional help and tutoring. Initially both Alex* and myself were worried about having a “stranger” entering the home. But the tutor’s help has been invaluable to us both. Sam* has been given lots of support in their learning and with their journey to College. Tutors are friendly, patient and encouraging and have shown Sam* that they are a good person and capable of having a positive future. “I feel very positive about the work that I am doing with Eastern Outreach. When I turned a certain age most of my funding was reduced or cut out, leaving me with nothing really to do and becoming stuck in the house most days. When Katy started working with me I instantly felt a change in my mood throughout the last two weeks due to the outings we have had. It is nice to build a bond with someone who I know won’t leave in a few weeks time, as that is what happens with most people I meet being in the care system. From meeting Sam and Katy I already knew this would be a massive positive change in my life. They were both so nice and supportive. I am really enjoying working with Eastern Outreach as they have helped my mental health and also made me feel supported, all while having a fun time!”

I have learnt how to use electrical circuits and lots of other science. I have learnt a lot of maths skills. I am more independent than I thought I was. I know what my goals are but it doesn’t feel like I am working towards goals because I am having so much fun.

Post 16 Transition FAQ’s

2 hr sessions, 3 times a week for 6 weeks is the learners schedule. This is carefully constructed to get the best out of each learner.

As well as all of the skills involved in the programme, learners will gain an Entry 3 ASDAN Lifestyle Challenge Certificate in "Developing Communication Skills" and in "Improving Functional Skills, Money & Numbers in an Enterprise Project".

Eastern Outreach can provide devices for learners to carry out the programme, so no, it is not a requirement.
