We know that life after Year 11 can feel overwhelming — especially if the traditional route isn’t the right fit. That’s where we come in. Our Post-16 Transition support is all about helping young people find the right next step, whether that’s college, sixth form, or an apprenticeship — and making sure they feel confident getting there.

From boosting functional skills and arranging work experience, to preparing for interviews and supporting EHCP reviews, we’re with them every step of the way. We don’t just point them in the right direction — we walk alongside them, offering personalised mentoring, guidance, and encouragement to make the whole process as smooth (and stress-free) as possible.

With our support, young people don’t just move on — they move forward, ready for whatever comes next.

Post 16 Transition
  • 2 hr sessions, 3 times a week for 6 weeks is the learners schedule. This is carefully constructed to get the best out of each learner.

  • As well as all of the skills involved in the programme, learners will gain an Entry 3 ASDAN Lifestyle Challenge Certificate in "Developing Communication Skills" and in "Improving Functional Skills, Money & Numbers in an Enterprise Project".

  • Eastern Outreach can provide devices for learners to carry out the programme, so no, it is not a requirement.
